If you have a shared frequency clause in your insurance plan, you will likely have to pay out of pocket for one out of the two.

In network with our office - Pay the patient portion for your treatment. The balance will be collected by our office after filing a claim. All quotes are estimates based on what we expect your insurance to cover.
Out-of-Network and Secondary Insurance - Pay upfront for your treatment. We will provide a claim form and receipt for you to file your claim for reimbursement.
